What Experts Say
Clindamycin vaginal cream is a first-line treatment for bacterial vaginosis in gynecology guidelines worldwide. The condition results from an overgrowth of anaerobic organisms that displace the normal Lactobacillus-dominant vaginal flora. Clindamycin suppresses these bacteria by inhibiting protein synthesis at the 50S ribosomal subunit, restoring microbial balance without systemic antibiotic exposure.
Gynecologists prefer the intravaginal route for clindamycin because it delivers high drug concentrations locally while limiting systemic absorption. This reduces the gastrointestinal side effects commonly associated with oral clindamycin, particularly antibiotic-associated colitis, and is well tolerated in most women including those who cannot take oral antibiotics.
Clinical evidence shows that a 7-night course of 2% clindamycin vaginal cream achieves cure rates comparable to oral metronidazole, the standard comparator in bacterial vaginosis trials. Completing the full course is essential even when symptoms resolve early, as incomplete treatment is a leading driver of recurrence.

Drug Interactions
Abwo Vaginal Cream may interact with certain drugs or products used concurrently. Inform your doctor or pharmacist if you are using any of the following.
- Erythromycin and macrolide antibiotics: May compete for the same ribosomal binding site, reducing the effectiveness of both agents. Avoid concurrent use.
- Latex contraceptive devices: The cream base may degrade latex. Avoid using latex condoms or diaphragms for at least 72 hours after completing treatment.
- Neuromuscular blocking agents: Clindamycin may enhance the effects of these agents if absorbed systemically. Inform your anesthesiologist if surgery is planned during or shortly after treatment.
Risks and warnings
- Antibiotic-associated colitis: Do not use if you have a history of antibiotic-associated diarrhea, Crohn's disease, or ulcerative colitis.
- Pregnancy: Consult your doctor before use during pregnancy. Use during the first trimester is not generally recommended without physician guidance.
- Allergy: Do not use if you are allergic to clindamycin, lincomycin, or any component of this cream.
- Latex devices: The cream base may weaken latex condoms and diaphragms. Avoid using latex-based products for at least 72 hours after completing treatment.

- Keep the tube sealed when not in use to maintain the consistency of the cream base.
- Do not refrigerate — cold temperatures alter the cream texture and make intravaginal application difficult.
- Dispose of each applicator after a single use. Store unused applicators in a clean, dry location until needed.
- Keep out of reach of children.
- Discard the pack if the cream has changed in color or odor, or if any applicator appears damaged or contaminated.
