- Most patients notice fever and pain settling within 48 to 72 hours of starting
- Can be taken with food, which makes a full course easier on the stomach
- Long safety record in adults and in children above one month of age
- Reaches useful levels in sputum, middle ear fluid and urine, so one drug covers several common sites
What Experts Say
Cefaclor is a second generation oral cephalosporin valued for its balance of gram positive and gram negative cover. It binds penicillin binding proteins in the bacterial cell wall and blocks the cross linking step, so the organism cannot hold its wall together. Physicians often reach for it when a throat, sinus or chest infection has not settled on amoxicillin alone.
Absorption after an oral dose is good, and the drug concentrates in sputum, middle ear fluid and urine. Its half-life is short, so the usual adult schedule is 250mg to 500mg every eight hours rather than once daily. Food slows absorption slightly but improves tolerance, a trade most doctors accept in patients prone to nausea.
Clearance is largely renal, so dose spacing matters in reduced kidney function and in older patients. Pakistan carries a high burden of self medication with antibiotics, and half finished courses drive resistance, so prescribers stress completing the full duration even after symptoms settle. A rash appearing in the second week, especially in a child, deserves review rather than a repeat prescription.

Drug Interactions
- Probenecid: Slows removal of cefaclor by the kidneys and raises its blood level.
- Warfarin and other oral anticoagulants: Bleeding tendency may increase, so clotting tests should be watched.
- Antacids with aluminium or magnesium: Taken together they can reduce how much cefaclor is absorbed.
- Aminoglycoside antibiotics and loop diuretics: Combined use can add strain on the kidneys, especially in older patients.
- Metformin: Cefaclor may raise metformin levels, so watch for stomach upset and low sugar symptoms.
- Oral typhoid and other live bacterial vaccines: The antibiotic can weaken the response to these vaccines.
Risks and warnings
- Penicillin allergy: Tell your doctor if any penicillin has ever caused a rash or swelling, as cross reaction with cefaclor is possible.
- Severe allergic reaction: Stop the capsule and seek emergency care if you develop facial swelling, wheezing or a spreading rash.
- Kidney impairment: Patients with reduced kidney function may need wider gaps between doses, so share any history of kidney disease.
- Antibiotic related diarrhoea: Watery or bloody stools during or after the course can signal a serious bowel infection and need medical review.
- Pregnancy and breastfeeding: Cefaclor is generally considered acceptable but should still be taken only on a doctor's advice.
- Prolonged use: Courses longer than ten days raise the risk of oral or vaginal thrush and resistant bacteria.
- Urine glucose testing: Copper based urine sugar tests can read falsely high, so tell the laboratory that you are on this antibiotic.
- Children: Paediatric dosing is based on body weight, so never use an adult 500mg capsule for a young child.
