Captopril belongs to the ACE inhibitor family and works by blocking the enzyme that produces angiotensin II, a hormone that tightens blood vessels. With less angiotensin II around, blood vessels relax and blood pressure comes down. It also reduces the workload on a failing heart.

Unlike many newer ACE inhibitors, captopril has a shorter duration of action and is usually taken two to three times a day rather than once daily. It should be taken on an empty stomach, about an hour before meals, since food can reduce how much of the drug is absorbed.

A dry, persistent cough is a well known side effect of this drug class, caused by a buildup of a substance called bradykinin. Regular blood tests are used to monitor potassium levels and kidney function, since captopril can raise potassium and affect kidney readings, especially in patients with existing kidney disease.
