What Experts Say
Atenolol works by selectively blocking beta 1 receptors in the heart, reducing heart rate, contractility and cardiac output. This lowers blood pressure and cuts myocardial oxygen demand, which is why the same drug class treats both hypertension and angina effectively.
Because atenolol is water soluble and cleared mainly by the kidneys rather than metabolized by the liver, doctors reduce the dose or extend the dosing interval in patients with reduced kidney function. Pakistan has a high prevalence of hypertension and ischemic heart disease, so cardioselective beta blockers remain a first line option in many treatment plans.
Stopping atenolol abruptly can trigger rebound high blood pressure, a faster heart rate or worsening angina, so physicians taper the dose gradually when treatment ends. Regular monitoring of pulse and blood pressure helps confirm the dose is controlling symptoms without causing excessive slowing of the heart rate.

Drug Interactions
- Calcium Channel Blockers (Verapamil, Diltiazem): Combined use can cause excessive slowing of the heart rate and low blood pressure
- Other Antihypertensives: Additive blood pressure lowering effect requires dose monitoring
- Insulin and Oral Diabetes Medicines: Atenolol can mask symptoms of hypoglycemia
- NSAIDs: Regular use may reduce the blood pressure lowering effect of atenolol
- Clonidine: Stopping clonidine while on atenolol can cause a dangerous rise in blood pressure
Risks and warnings
- Asthma and COPD: Use with caution since beta blockers can worsen airway narrowing in susceptible patients
- Heart Block: Avoid in people with significant heart block or a very slow heart rate unless a pacemaker is fitted
- Diabetes: May mask warning signs of low blood sugar such as a fast heartbeat
- Kidney Impairment: Dose adjustment is needed since atenolol is mainly cleared by the kidneys
- Sudden Discontinuation: Stopping abruptly can trigger rebound high blood pressure or worsening angina
- Pregnancy: Use only if clearly needed and under close medical supervision
