What Experts Say
Chlorpheniramine is a first generation antihistamine with strong anticholinergic activity, which explains why it dries nasal secretions and reduces sneezing more effectively than newer non sedating antihistamines. Dextromethorphan acts centrally on the cough reflex in the brainstem, suppressing dry cough without the respiratory depression risk linked to opioid based antitussives.
Physicians titrate the dose strictly by age and body weight because first generation antihistamines carry a narrower safety margin in young children, with sedation and occasional paradoxical excitation being the main effects monitored. Dosing intervals are kept at least six hours apart to avoid stacking sedative effects in a small body mass.
Pakistan carries a high seasonal burden of pediatric upper respiratory infections linked to pollution and rapid weather shifts, so this combination remains a common first line choice for short term symptom control. Doctors across Pakistan caution against prolonged use, since masking a persistent cough can delay diagnosis of an underlying chest infection.
