What Experts Say
Bempeget-Ez combines bempedoic acid and ezetimibe, two lipid lowering agents that work through different pathways. Bempedoic acid blocks ATP citrate lyase, an enzyme the liver uses to make cholesterol, while ezetimibe blocks absorption of cholesterol from the intestine. Used together, they produce a greater reduction in LDL cholesterol than either drug alone.
Because bempedoic acid is activated mainly in the liver and not in muscle tissue, it carries a lower risk of the muscle pain seen with statins, which makes this combination a useful option for patients who cannot tolerate higher statin doses. Clinical trials have shown meaningful LDL reduction and a lower rate of major cardiovascular events in appropriate patients.
With rising rates of heart disease and high cholesterol among adults in Pakistan, physicians increasingly use combination lipid lowering therapy rather than relying on a single agent. Regular monitoring of uric acid and liver function is advised during treatment, since gout flares and mild liver enzyme changes have been reported with bempedoic acid use.
Drug Class

Drug Interactions
- Simvastatin And Pravastatin: Bempedoic acid can raise blood levels of these statins, increasing the risk of muscle related side effects, so dose limits apply.
- Cyclosporine: Levels of both bempedoic acid and ezetimibe may rise when combined with cyclosporine, requiring close monitoring.
- Fibrates: Combined use with fenofibrate or gemfibrozil can enhance lipid lowering but may raise the risk of gallstones.
- Bile Acid Sequestrants: Cholestyramine and similar resins reduce ezetimibe absorption, so doses should be separated by two to four hours.
- Warfarin: Ezetimibe may enhance the effect of warfarin, so INR should be monitored closely when starting or stopping therapy.
Risks and warnings
- Prescription Only: This medicine should be used strictly under a doctor's supervision, not for self medication.
- Gout Risk: Uric acid levels may rise and trigger gout, usually within the first four weeks of treatment.
- Tendon Injury: The risk of tendon rupture can increase, especially in patients over 60, those on corticosteroids or fluoroquinolone antibiotics, or with a prior tendon problem.
- Liver Function: Mild elevation in liver enzymes may occur, so periodic liver tests are advised.
- Pregnancy And Breastfeeding: Not recommended, as safety in pregnancy and lactation has not been established.
- Allergy Risk: Known hypersensitivity to bempedoic acid or ezetimibe is a reason to avoid this medicine.
