What Experts Say

Betamethasone sodium phosphate is a potent synthetic corticosteroid that reduces inflammation by suppressing the release of chemical mediators responsible for swelling, redness and discomfort in the eye, ear and nasal tissue. It acts quickly, which is why it is paired with an antibiotic whenever infection is also suspected.

Neomycin sulfate is a broad spectrum aminoglycoside antibiotic effective against many gram negative and some gram positive bacteria that commonly cause external eye, ear and nasal infections. Because it works on the surface rather than through the bloodstream, it delivers a high local concentration while limiting systemic absorption when used as directed.

Doctors in Pakistan generally reserve steroid antibiotic combinations for short courses under close supervision, since prolonged steroid use on the eye can raise pressure inside the eye and mask a worsening infection. A proper examination before starting therapy helps confirm the diagnosis and rules out conditions such as corneal ulcers where steroids are unsuitable.

Drug Interactions

  • Other topical steroids: Combining with additional steroid eye, ear or nose products can increase side effect risk.
  • Live vaccines: Corticosteroids may reduce the effectiveness of live vaccines given around the same time.
  • Other ototoxic drugs: Using with other ear-toxic medicines such as certain injectable antibiotics may raise the risk of hearing damage.
  • Other eye medicines: Space out other eye drops by several minutes to avoid diluting the effect.

Risks and warnings

  • Prolonged use: Extended use in the eye can raise intraocular pressure and increase glaucoma risk.
  • Cataract risk: Long-term steroid use in the eye may contribute to cataract formation.
  • Delayed healing: May slow healing of eye or ear tissue and mask signs of a worsening infection.
  • Viral or fungal infection: Not suitable for viral, fungal or tubercular infections of the eye or ear.
  • Perforated eardrum: Avoid ear use if the eardrum is perforated, since neomycin can affect hearing.
  • Corneal damage: Avoid use with corneal ulcers or significant corneal thinning without medical supervision.
  • Children: Use in infants and young children only under medical guidance.
