What Experts Say
Esomeprazole is the S isomer of omeprazole and belongs to the proton pump inhibitor class. It binds irreversibly to the hydrogen potassium ATPase enzyme, commonly called the acid pump, in the gastric parietal cells. This blocks the final step of acid production regardless of what stimulates it, giving more complete and longer lasting suppression than many older acid reducing agents.
Gastroenterologists generally reserve the 40mg strength for moderate to severe erosive esophagitis, larger ulcers, and combination regimens used to eradicate Helicobacter pylori infection. Healing rates for erosive esophagitis are consistently high after four to eight weeks of therapy at this dose, with faster symptom relief than standard strength options.
Doctors in Pakistan increasingly caution against long term self medication with high dose PPIs bought without a prescription. Extended use can mask more serious conditions such as bleeding ulcers, so a repeat consultation is advised if symptoms persist beyond eight weeks.
