What Experts Say
Piroxicam blocks cyclooxygenase enzymes that generate prostaglandins, the chemicals responsible for pain, swelling and fever during inflammation. Betadex complexation wraps the piroxicam molecule inside a cyclodextrin ring, improving its solubility so it dissolves and gets absorbed faster than plain piroxicam.
Piroxicam has a long half life, so a single daily dose keeps working through the day. This same property means the drug can build up in the body with repeated use, which is why health authorities have restricted it to second line treatment at the lowest effective dose for the shortest duration, particularly in older adults.
Doctors usually reserve piroxicam for patients who have not responded to safer NSAIDs, because of its links to stomach irritation and rare serious skin reactions. Kidney function and gastrointestinal symptoms need regular monitoring during extended use, and the drug is avoided in anyone with a history of ulcers or bleeding problems.
