  • Those with a history of bronchospasm triggered by previous mucolytic treatments

Drug Interactions

  • Anticoagulants (warfarin, heparin): Carbex syrup may increase the risk of bleeding when taken with blood-thinning medicines.
  • Cough suppressants (codeine-based syrups): These reduce mucus clearance and work against the action of carbocisteine.
  • Antibiotics (amoxicillin, ciprofloxacin, doxycycline): Carbocisteine may interact with certain antibiotics; inform your doctor before combining treatments.
  • Bronchodilators: No direct contraindication, but your doctor should be aware of all respiratory medicines being used together.

Risks and warnings

  • Gastrointestinal risk: Carbex syrup can weaken the stomach lining; patients with a history must use it only under medical supervision.
  • Active GI bleeding: Stop this medicine immediately and seek medical care if any signs of stomach or intestinal bleeding appear.
  • Asthma: Patients with bronchial asthma should use carbocisteine with caution, as it may occasionally trigger bronchospasm.
  • Kidney impairment: Dose adjustment may be needed in patients with moderate-to-severe kidney disease.
  • Liver disease: Patients with liver conditions should be monitored closely during treatment.
  • Diabetes: The syrup formulation contains sucrose; patients with diabetes should discuss safer alternatives with their doctor.
  • Pregnancy: Not recommended during the first trimester; use in later pregnancy only when clearly necessary.
