About Cefstar Injection 500mg
Cefstar is a fourth-generation cephalosporin antibiotic injection for severe bacterial infections.
Cefstar Injection 500mg Uses
- Treatment of moderate to severe pneumonia, including hospital acquired chest infections
- Complicated and uncomplicated urinary tract infections caused by sensitive bacteria
- Skin and soft tissue infections that do not settle with oral antibiotics
- Complicated infections inside the abdomen, given together with an anti-anaerobic antibiotic
- Starting treatment in patients who have fever with a very low white cell count
- Infections caused by Pseudomonas aeruginosa and other resistant gram-negative bacteria
Cefstar Injection 500mg Benefits
- Covers many gram-positive and gram-negative bacteria with a single antibiotic
- Stays stable against several beta-lactamase enzymes that destroy older cephalosporins
- Reaches high blood levels within minutes because it is given directly into a vein or muscle
- Can be given by the intravenous or the intramuscular route, so it suits both ward and clinic settings
- Safe to use in children when the dose is calculated from body weight
- Allows a switch to oral antibiotics once the infection starts settling
What Experts Say
Cefepime is a fourth-generation cephalosporin, and its strength comes from a zwitterionic structure that helps it cross the outer membrane of gram-negative bacteria quickly while resisting many chromosomal AmpC beta-lactamases. That combination gives dependable activity against Pseudomonas aeruginosa and Enterobacter species that earlier cephalosporins often miss.
Killing depends on the time the drug level stays above the minimum inhibitory concentration rather than on the peak level. Physicians therefore prefer every 8 to 12 hour schedules, and extended infusions in critically ill patients. The 500mg vial fits milder urinary infections and weight-based paediatric dosing, while severe or pseudomonal disease usually needs the 1g or 2g strength.
Almost all of the drug leaves the body through the kidneys, so dose reduction in reduced creatinine clearance is essential. Ignoring this can cause cefepime neurotoxicity, seen as confusion, muscle jerks or seizures. Pakistan carries a high burden of resistant hospital infections, so doctors reserve this antibiotic for culture-guided use.
Drug Class

How to use Cefstar Injection 500mg?
- Cefstar Injection 500mg is given only by a doctor, nurse or trained health worker
- The dry powder in the vial is mixed with sterile water or another approved diluent just before use
- Doses are usually repeated every 8 to 12 hours, depending on how severe the infection is
- An intravenous dose is pushed slowly over about 3 to 5 minutes or run as a short drip
- Children are given a dose worked out from body weight, never the standard adult dose
- Tell the doctor about any kidney problem so the dose and the gap between doses can be adjusted
- Complete the full prescribed course even if the fever settles after two or three days
Drug Interactions
- Aminoglycosides such as amikacin: The combined load on the kidneys and inner ear raises the risk of kidney damage and hearing loss.
- Loop diuretics such as furosemide: Using both together can increase the chance of kidney injury, so renal function needs checking.
- Oral anticoagulants such as warfarin: Bleeding risk may rise, and the INR should be monitored more closely during therapy.
- Probenecid: It slows removal of cefepime by the kidneys and pushes blood levels of the antibiotic higher.
- Live vaccines such as oral typhoid or BCG: The antibiotic can blunt the immune response to these vaccines.
- Metronidazole and aminoglycoside infusions: These should never be mixed in the same syringe or drip line because of physical incompatibility.
Risks and warnings
- Penicillin allergy: Patients who have had a severe reaction to penicillin or any cephalosporin may develop rash, swelling or anaphylaxis with this injection.
- Kidney impairment: The dose must be reduced when creatinine clearance falls, because the drug builds up quickly in the blood.
- Neurotoxicity: Confusion, drowsiness, muscle jerks or seizures can appear in elderly patients and in those with poor kidney function.
- Severe diarrhoea: Watery or bloody stools during or after treatment may signal Clostridioides difficile colitis and need urgent review.
- Pregnancy and breastfeeding: Use only when the doctor decides the benefit is clearly greater than the possible risk to the baby.
- Antibiotic resistance: Using this injection for viral fevers or flu adds no benefit and helps resistant bacteria spread.
- Laboratory interference: It can give a false positive urine glucose test and a positive direct Coombs test without haemolysis.
- Injection site care: Repeated doses in the same site may cause pain, hardening or inflammation of the vein.
