What Experts Say
Clomiphene citrate is a selective estrogen receptor modulator. It blocks estrogen receptors in the hypothalamus, so the brain reads estrogen levels as low. The pituitary gland then releases more FSH and LH into the bloodstream. These hormones push ovarian follicles to grow and release a mature egg, which restores the chance of natural conception.
Treatment usually starts at 50mg once daily for five days, beginning between day 2 and day 5 of the menstrual cycle. Ovulation is expected 5 to 10 days after the last tablet. If no egg is released, a specialist may raise the dose to 100mg in the next cycle. Most guidelines advise stopping after about six ovulatory cycles.
Specialists confirm response with follicle tracking ultrasound and mid-luteal progesterone testing. Polycystic ovary syndrome is a leading cause of anovulatory infertility among women in Pakistan, which keeps demand for ovulation induction high. Careful monitoring screens for ovarian cysts and lowers the risk of multiple pregnancy and hyperstimulation.

Drug Interactions
- Ospemifene: Both medicines act on estrogen receptors, so the combination is avoided.
- Gonadotropin injections (FSH, hMG, hCG): Additive ovarian stimulation raises the risk of hyperstimulation and multiple pregnancy.
- Hormonal contraceptives and estrogen products: These oppose ovulation induction and can blunt the response to clomiphene.
- JAK inhibitors (tofacitinib, baricitinib, ruxolitinib): Combined use is flagged for a higher risk of blood clots.
- Eltrombopag: This platelet stimulating drug is flagged for added clot risk when given with clomiphene.
- Fluoroestradiol F 18: Clomiphene can interfere with the results of this estrogen receptor imaging scan.
Risks and warnings
- Multiple pregnancy: Twins occur in roughly 7 to 8 of every 100 clomiphene pregnancies, so counseling is essential.
- Ovarian hyperstimulation: Severe stomach pain, bloating, rapid weight gain, or breathlessness needs urgent medical care.
- Visual disturbances: Stop the tablets and see a doctor if blurring, spots, or flashes appear, as symptoms can persist.
- Pregnancy check: A doctor must rule out pregnancy before every new course, since the drug is not for use in pregnancy.
- Liver disease: Do not use with active liver disease or a history of liver dysfunction.
- Ovarian cysts: Large cysts other than polycystic ovaries must be excluded before each cycle.
- Unexplained bleeding: Abnormal uterine bleeding needs full evaluation before starting treatment.
