What Experts Say
Lisinopril is an ACE inhibitor that blocks the enzyme converting angiotensin I into angiotensin II, a hormone that narrows blood vessels and raises blood pressure. Hydrochlorothiazide is a thiazide diuretic that helps the kidneys remove excess sodium and water. Together they lower blood pressure through two separate mechanisms.
This fixed dose combination is generally used when blood pressure is not controlled by a single medicine. Regular monitoring of kidney function, blood potassium, and blood pressure is recommended, since ACE inhibitors can raise potassium while thiazide diuretics can lower it. A persistent dry cough is a known ACE inhibitor side effect.
This medicine is avoided during pregnancy, since ACE inhibitors can cause serious harm to a developing baby, especially in the second and third trimester. People with a history of ACE inhibitor related swelling of the face or throat should not use it. Diabetic patients and older adults benefit from closer monitoring of blood sugar, kidney function, and electrolyte levels.

Drug Interactions
- Potassium supplements or potassium sparing diuretics: Combined use can raise blood potassium to dangerous levels.
- NSAIDs (ibuprofen, diclofenac): Regular use can reduce the blood pressure lowering effect and increase the risk of kidney problems.
- Lithium: Blood lithium levels can rise, increasing the risk of toxicity.
- Other blood pressure medicines or diuretics: Additive effects can cause blood pressure to drop too low.
- Diabetes medicines: Hydrochlorothiazide can raise blood sugar, so glucose levels need closer monitoring.
Risks and warnings
- Pregnancy: Not used during pregnancy because ACE inhibitors can cause serious harm or death to the unborn baby.
- Angioedema history: Avoided in anyone with a past reaction of facial or throat swelling to an ACE inhibitor.
- Kidney function: Used cautiously in kidney disease or narrowing of the kidney arteries, with regular monitoring.
- Potassium levels: Blood potassium should be checked periodically, since ACE inhibitors can raise it while thiazides can lower it.
- Sulfa sensitivity: Hydrochlorothiazide is a sulfonamide derivative, so caution is needed in people with sulfa drug allergies.
- Diabetes and gout: Blood sugar and uric acid levels may rise, so monitoring is advised in these patients.
- Sudden stopping: This medicine is not stopped abruptly without medical advice, as blood pressure can rebound.
- Dehydration: Excess fluid loss from vomiting, diarrhea, or heavy sweating can increase the risk of low blood pressure.
