What Experts Say
Physicians reserve Co-Valtec Tablet 25mg / 160mg for patients with resistant or difficult to control hypertension who need the maximum available doses of both the ARB and the diuretic component working together in a single tablet.
The higher Hydrochlorothiazide dose of 25mg produces a more pronounced natriuretic effect than the 12.5mg strength, while Valsartan 160mg gives fuller angiotensin II receptor blockade. Together this offers a stronger drop in blood pressure than lower dose combinations, but it also raises the chance of symptomatic hypotension, dizziness and electrolyte shifts, particularly early in treatment.
Doctors typically move patients to this strength only after an adequate trial of a lower dose combination has fallen short, and recommend closer monitoring of blood pressure, kidney function and serum potassium once treatment begins at this level, especially in older adults, people with kidney disease, or anyone already taking other blood pressure lowering medicines.

Drug Interactions
- NSAIDs: pain relievers like ibuprofen or diclofenac can reduce the blood pressure lowering effect and raise kidney risk, more relevant at this higher strength
- Potassium supplements or potassium sparing diuretics: combined use can push blood potassium to unsafe levels
- Lithium: this combination can raise lithium levels in the blood, increasing the risk of toxicity
- Other blood pressure medicines: additive effect raises the chance of excessive hypotension at this dose
- ACE inhibitors: avoid combining, dual blockade of the same hormone system raises the risk of kidney injury and high potassium
- Digoxin: low potassium from the higher Hydrochlorothiazide dose can increase the risk of digoxin toxicity
Risks and warnings
- Pregnancy: avoid during pregnancy, especially the second and third trimester, since ARBs can cause fetal harm
- Symptomatic hypotension: higher risk at this strength, especially after the first few doses or in dehydrated patients
- Kidney impairment: use with caution, this high strength combination needs closer renal function monitoring
- Electrolyte imbalance: the 25mg Hydrochlorothiazide dose carries a greater risk of low potassium and low sodium than lower strengths
- Sulfa sensitivity: Hydrochlorothiazide is a sulfonamide derivative, use caution if you have a known sulfa allergy
- Diabetes and gout: thiazide diuretics can raise blood sugar and uric acid levels, monitor closely at this dose
- Elderly patients: start under close medical supervision, since the higher dose raises fall risk from low blood pressure
