What Experts Say
When two antihypertensive medicines fail to control blood pressure, doctors often add a third agent from a different drug class rather than raising the dose of one medicine. A calcium channel blocker, an angiotensin receptor blocker and a thiazide diuretic combined in one tablet target three separate mechanisms that raise blood pressure.
The calcium channel blocker component relaxes artery muscle by limiting calcium entry into vascular cells, lowering resistance to blood flow. The angiotensin receptor blocker component blocks angiotensin II from narrowing vessels and prompting the kidneys to hold sodium. The thiazide diuretic component helps the kidneys remove extra sodium and fluid, reducing the volume the heart circulates.
Each drug lowers blood pressure through an independent route, so the combined effect is generally stronger than any single medicine alone, while each component can be dosed lower than it would need alone. Regular checks of blood pressure, kidney function and electrolyte levels are recommended, particularly in the first weeks after starting or adjusting the dose.
