What Experts Say
Perindopril and Amlodipine work through two complementary pathways to lower blood pressure. Perindopril inhibits the angiotensin-converting enzyme, reducing angiotensin II and easing pressure on artery walls. Amlodipine blocks calcium entry into vascular smooth muscle, producing direct arterial vasodilation. Together they achieve a steadier reduction than either drug alone.
The 8mg/10mg strength is the highest approved dose combination of this pairing. Doctors reserve it for patients whose blood pressure stays above target on lower combinations such as 4mg/5mg or 8mg/5mg. It also suits patients with more severe or long standing hypertension who need a stronger effect. The higher Amlodipine dose adds vasodilation when moderate strengths fall short.
This strength calls for closer clinical monitoring during treatment. Renal function and serum potassium levels need periodic checks, since ACE inhibitors can affect kidney filtration and potassium balance. The 10mg Amlodipine dose carries a higher chance of ankle swelling and flushing than lower strengths. Blood pressure response and tolerability guide whether the dose continues or is adjusted.

Drug Interactions
- Potassium Sparing Diuretics: Combination with spironolactone, amiloride, or potassium supplements increases the risk of hyperkalemia.
- NSAIDs: Reduce the blood pressure lowering effect and may impair kidney function when combined with Perindopril.
- Lithium: ACE inhibitors can raise lithium levels, increasing the risk of toxicity.
- Other Antihypertensives: Additive blood pressure lowering effect that may require dose adjustment.
- CYP3A4 Inhibitors: Drugs such as ketoconazole or ritonavir can raise Amlodipine blood levels.
- Simvastatin: Amlodipine can raise simvastatin levels, increasing the risk of muscle related side effects at higher statin doses.
Risks and warnings
- Pregnancy: Contraindicated in pregnancy because ACE inhibitors can cause fetal and neonatal injury or death.
- Hypotension: May cause a significant drop in blood pressure, especially with the first dose or in volume depleted patients.
- Renal Function: Requires periodic monitoring of kidney function, particularly in patients with pre existing renal impairment.
- Angioedema: Rare but serious swelling of the face, lips, tongue, or throat that needs immediate medical attention.
- Hyperkalemia: Can raise blood potassium levels, especially when combined with potassium sparing agents.
- Peripheral Edema: Higher risk of ankle and leg swelling at the 10mg Amlodipine dose compared to lower strengths.
- Aortic Stenosis: Use with caution in patients with severe aortic stenosis due to risk of reduced coronary perfusion.
- Hepatic Impairment: Requires dose caution and monitoring in patients with significant liver disease.
