What Experts Say
Dermatologists in Pakistan often favor topical terbinafine for dermatophyte infections because it is fungicidal rather than just fungistatic against most tinea causing fungi. This means it actively kills fungal cells instead of only halting their growth, which usually shortens treatment to one or two weeks compared with the four weeks sometimes needed with older azole creams.
Pakistan carries a heavy burden of tinea infections, driven by a hot humid climate, tight synthetic clothing and shared bathing spaces that favor fungal growth. Clinicians often see patients stop treatment as soon as itching and redness fade, well before the fungal load clears fully, which is a common reason for relapse and reinfection in general practice.
Terbinafine blocks squalene epoxidase, an enzyme the fungus needs to build its cell membrane, causing a toxic buildup of squalene inside the fungal cell. This direct toxicity, on top of membrane disruption, explains why symptom relief tends to come faster than with azole based creams. Most physicians advise continuing the cream for one to two weeks after symptoms clear to prevent recurrence.
