What Experts Say
Hydrochlorothiazide works by blocking sodium reabsorption in the distal kidney tubule, which increases urine output and lowers blood volume. Doctors across Pakistan often start with the lowest effective dose to control blood pressure while limiting electrolyte disturbance in long-term therapy.
Regular monitoring of serum potassium, sodium and uric acid is standard practice with thiazide therapy, since even low doses carry a small risk of hypokalemia or raised blood sugar. Physicians recommend periodic blood tests, especially in older adults and patients on combination antihypertensive therapy.
Patients with diabetes, gout or kidney impairment need individualised dosing, since hydrochlorothiazide can affect glucose tolerance and uric acid clearance. Physicians recommend pairing it with lifestyle changes such as reduced salt intake and regular exercise for better long-term blood pressure control.
