What Experts Say
Perindopril belongs to the ACE inhibitor class, which works by blocking the enzyme that converts angiotensin I into angiotensin II, a hormone that narrows blood vessels. By reducing this conversion, blood vessels relax and blood pressure falls, while the heart faces less resistance when pumping blood.
In heart failure, this reduced vascular resistance eases the workload on a weakened heart muscle, which is why perindopril is often combined with other heart failure medicines rather than used alone. Blood pressure response is usually evident within one to two weeks of consistent daily dosing.
Clinicians routinely check kidney function and blood potassium levels before starting perindopril and periodically afterward, since ACE inhibitors can affect both. A dry cough is a recognized class effect linked to increased bradykinin levels, and switching to a different blood pressure drug class is considered if it becomes bothersome.
