What Experts Say
Doctors across Pakistan usually start memantine therapy at a low dose and increase it gradually over several weeks rather than beginning at the full maintenance strength. This stepwise titration reduces the chance of dizziness, headache, confusion or agitation that can occur if the NMDA receptor blockade is introduced too quickly in an elderly patient.
Physicians recommend checking kidney function before starting treatment, since memantine is cleared mainly through the kidneys rather than the liver. Patients with moderate to severe renal impairment often need a reduced maintenance dose, and regular monitoring helps prevent drug accumulation and unwanted central nervous system effects over long term use.
Clinicians view memantine as a way to regulate excess glutamate signalling that contributes to nerve cell damage in Alzheimer's disease, rather than simply boosting acetylcholine the way older dementia medicines do. This distinct mechanism makes it a useful add on for patients already using a cholinesterase inhibitor for additional cognitive benefit.

Drug Interactions
- Amantadine: Combining with amantadine increases the risk of central nervous system toxicity because both drugs act on NMDA receptors.
- Ketamine: Concurrent use may increase the risk of psychiatric side effects due to overlapping NMDA receptor activity.
- Dextromethorphan: Found in many cough syrups, this combination can raise the risk of confusion and hallucinations.
- Other NMDA Antagonists: Avoid combining with other NMDA receptor antagonists to reduce the risk of additive central nervous system effects.
- Drugs That Alkalinize Urine: Sodium bicarbonate and similar agents can reduce memantine clearance and increase blood levels.
Risks and warnings
- Renal Impairment: Dose adjustment is needed in patients with moderate to severe kidney disease because memantine is excreted mainly by the kidneys.
- Seizure History: Use with caution in patients with a history of seizures as memantine can lower the seizure threshold.
- Elderly Patients: Older adults are more prone to dizziness and confusion, so falls risk should be monitored closely.
- Driving and Operating Machinery: Memantine can cause dizziness and drowsiness, so avoid driving until you know how the medicine affects you.
- Pregnancy and Breastfeeding: Use only if clearly needed and under specialist supervision, as safety data in pregnancy is limited.
