How to use Esofast capsule 40mg?
- The usual adult dose is one 40mg capsule once daily, taken about an hour before a meal.
- A whole capsule swallow with a glass of water works best. Crushing the capsule reduces its effect.
- Doctors sometimes extend GERD treatment to four to eight weeks depending on symptom severity.
- H. pylori treatment plans usually pair this capsule with two antibiotics for seven to fourteen days.
- Consistent daily timing helps keep stomach acid levels steady through the day.
Drug Interactions
- Clopidogrel: This capsule can weaken clopidogrel's blood thinning action, a key concern for heart stent patients.
- Warfarin: Combined use can increase bleeding tendency and needs INR checks.
- Methotrexate: High dose methotrexate clearance slows, raising toxicity risk.
- Rifampicin: This antibiotic lowers esomeprazole blood levels and reduces its effect.
- Ketoconazole and Itraconazole: Reduced stomach acid lowers absorption of these antifungal medicines.
- Tacrolimus: Blood levels of this transplant medicine can rise, needing closer monitoring.
Risks and warnings
- Kidney or Liver Disease: A lower dose or closer monitoring may be needed for patients with reduced organ function.
- Pregnancy and Breastfeeding: Use only under a doctor's supervision, with the lowest effective dose preferred.
- Bone Health: Extended use raises fracture risk in the hip, wrist or spine, a concern for older Pakistani patients with limited dairy intake and low vitamin D levels.
- Low Magnesium: Treatment beyond three months can lower blood magnesium, leading to muscle spasms or heart rhythm changes.
- Clostridioides difficile Infection: Reduced stomach acid slightly raises the risk of this intestinal infection, especially after antibiotic use.
