What Experts Say
Esomeprazole is the S isomer of omeprazole and works as an irreversible proton pump inhibitor. It binds to the hydrogen potassium ATPase enzyme system on gastric parietal cells, blocking the final step of acid secretion. This action produces a more predictable and sustained reduction in gastric acid than earlier generation PPIs.
Clinical trials show esomeprazole heals erosive esophagitis in most patients within four to eight weeks and maintains healing better than comparable agents. Its pharmacokinetics allow once daily dosing to keep intragastric pH above four for a longer duration, which supports mucosal healing and symptom control in gastroesophageal reflux disease.
Esomeprazole undergoes hepatic metabolism through the CYP2C19 pathway, so genetic variation can affect individual response and drug interaction risk. Long term suppression of gastric acid alters mineral and vitamin absorption, which is why clinicians periodically reassess the need for continued therapy beyond the initial treatment course.
