What Experts Say
Gastroenterologists commonly prescribe esomeprazole 20mg as a first-line option for GERD because it maintains a more stable acid suppression profile across the day compared to older PPIs, reducing breakthrough symptoms at night. Doctors usually recommend taking it on an empty stomach, about an hour before the first meal of the day, since food delays its absorption and blunts its effect.
Acid-related disorders including GERD, gastritis, and peptic ulcer disease are extremely common in Pakistan, driven by spicy diets, irregular meal timing, high tobacco use, and widespread self-medication with NSAIDs for pain. Physicians across Pakistan frequently see patients who have been using over-the-counter antacids for months without addressing the underlying acid disorder, which delays proper PPI-based treatment.
For H. pylori eradication, esomeprazole is typically combined with two antibiotics for one to two weeks rather than used alone, since PPI monotherapy does not clear the infection. Doctors also caution that stopping a PPI abruptly after weeks of use can trigger rebound acid hypersecretion, so tapering is often advised for anyone who has taken it for more than a few weeks.
