What Experts Say

Iron deficiency anemia is one of the most common nutritional deficiencies among women and children in Pakistan, driven by low dietary iron intake, chronic blood loss, and repeated pregnancies. Left untreated, it causes persistent fatigue, poor concentration, and reduced immunity, making early correction with an oral iron supplement an important part of routine care.

Ferrous sulphate is one of the most studied and cost effective forms of oral iron, delivering elemental iron that the body absorbs mainly in the upper small intestine. Absorption improves with vitamin C rich foods and declines sharply with tea, coffee, calcium, or antacids, so spacing intake from these is usually advised. A slow release capsule format also eases the gastrointestinal discomfort that often limits adherence with plain iron salts.

Folic acid is paired with iron because pregnancy sharply increases the body's requirement for both nutrients to support the expanding blood volume and the developing fetus. Adequate folic acid intake before and during early pregnancy is linked to a lower risk of neural tube defects, which is why combination iron and folic acid supplements are widely recommended for women who are pregnant or planning conception.

How to use Fefol Capsule?

  • Take one capsule daily, or as directed by your doctor
  • Swallow the capsule whole with a full glass of water
  • Take with or after food if it causes stomach upset
  • Avoid taking with tea, coffee, milk, or calcium supplements, as they reduce iron absorption
  • Take at least two hours apart from antacids or antibiotics
  • Do not crush or open the capsule
  • Continue the full course advised by your doctor, even if symptoms improve

Drug Interactions

  • Antacids: Reduce iron absorption significantly, so space dosing by at least two hours.
  • Tetracycline and Quinolone Antibiotics: Iron binds with these antibiotics, lowering the effectiveness of both, so separate dosing times.
  • Levothyroxine: Iron can reduce absorption of thyroid medicine when taken together.
  • Calcium Supplements: Compete with iron for absorption in the gut, reducing effectiveness of both.
  • Proton Pump Inhibitors and H2 Blockers: Reduced stomach acid from these medicines can lower iron absorption.

Risks and warnings

  • Gastrointestinal Upset: Nausea, cramping, or constipation are common and usually mild with the slow release formula.
  • Accidental Overdose in Children: Iron overdose can be serious or fatal in young children, so keep the pack well out of their reach.
  • Iron Overload: Avoid use in hemochromatosis or other conditions of existing iron overload unless a doctor advises otherwise.
  • Peptic Ulcer or Bowel Disease: Use with caution if you have active ulcers, colitis, or diverticulitis, since iron can irritate the gut lining.
  • Pregnancy and Breastfeeding: Widely recommended in pregnancy, but confirm the right dose with your doctor rather than self prescribing.
  • Stool Discoloration: Dark or greenish stools are a harmless and expected effect of the iron content.
