What Experts Say
Codeine is an opioid the body converts into morphine in the liver. In this combination it works with paracetamol and caffeine to relieve pain that plain paracetamol or ibuprofen cannot fully control. Doctors reserve opioid combinations like this for short courses because pain relief must be weighed against dependence risk.
Paracetamol has a firm daily dose ceiling because taking too much, especially with alcohol or on an empty stomach, can cause serious liver injury. Caffeine is added in a small amount because it can boost how well paracetamol works, though it can also disturb sleep or cause palpitations in large amounts. Clinicians generally advise the lowest effective dose for the shortest possible time.
Most guidance limits codeine containing tablets to three to five days unless a doctor extends treatment after review. Prolonged use raises the chance of constipation, tolerance and withdrawal on stopping. Anyone whose pain continues beyond this window, or who needs rising doses for the same relief, should see a doctor rather than continue self medicating.
