What Experts Say
Fluconazole works by blocking an enzyme fungal cells need to build their outer membrane. Without this enzyme, the fungal cell wall becomes weak and leaky, and the organism eventually dies. Because human cells do not rely on this same enzyme in the same way, fluconazole is generally well tolerated at standard doses.
In children, dosing is calculated by body weight rather than a fixed amount, which is why the suspension form is preferred over tablets for younger patients. A typical starting dose ranges from 3 to 12 mg per kg once daily, depending on the type and severity of the fungal infection being treated. The oral suspension allows more accurate weight based measurement in infants and toddlers who cannot swallow capsules.
Because fluconazole is cleared mainly through the kidneys, children with reduced kidney function may need a lower dose or longer interval between doses. Doctors also watch for changes in liver enzymes and heart rhythm during longer treatment courses, and they generally prescribe the shortest duration that clears the infection effectively.
