What Experts Say
Montelukast Sodium works as a selective leukotriene receptor antagonist. It blocks leukotriene D4 action on the CysLT1 receptors lining the airways, reducing bronchoconstriction, mucus secretion and inflammation. This mechanism makes it useful for both asthma prevention and allergic rhinitis control, unlike bronchodilators that only relieve symptoms already present.
In pediatric practice across Pakistan, oral granules are often preferred over tablets for children who cannot swallow solid dosage forms. The 4mg strength is prescribed for younger children based on weight and age, taken once daily in the evening as maintenance therapy rather than for acute asthma attacks.
Regulatory bodies have flagged montelukast for a small risk of neuropsychiatric effects such as mood changes, sleep disturbances or vivid dreams, so caregivers should watch for behavioral changes after starting therapy. It should never replace a rescue inhaler during an asthma attack. Doctors usually reassess response after four to six weeks.
