What Experts Say
Once a patient tolerates the starting dose well, physicians commonly move to 300mg as the standard maintenance strength for both epilepsy and neuropathic pain. This dose sits within the range shown to give meaningful pain relief and seizure control while keeping side effects manageable for most adults.
Gabapentin's bioavailability falls as the dose rises because absorption depends on a saturable gut transporter, which is why doctors often split the daily total into two or three doses rather than giving it all at once. This pattern keeps blood levels steadier and reduces peak related side effects such as sedation and dizziness.
Since the kidneys clear gabapentin almost unchanged, physicians recalculate the maintenance dose whenever kidney function changes, particularly in older adults or those with diabetes related kidney disease, a common combination given the high burden of diabetes across Pakistan. Follow up visits let doctors confirm the 300mg dose is still appropriate and watch for signs of accumulation such as drowsiness or confusion.

Drug Interactions
- Opioids: Combined use significantly raises the risk of severe sedation and slowed breathing.
- Antacids containing aluminum or magnesium: These reduce absorption if taken at the same time, so space doses at least two hours apart.
- Alcohol: Drinking alcohol worsens drowsiness and coordination problems.
- Other CNS depressants: Sleep medicines, muscle relaxants and anti anxiety drugs add to sedation.
- Morphine: Concurrent use can raise gabapentin levels and intensify side effects.
Risks and warnings
- Drowsiness: This maintenance strength can cause marked sleepiness, so avoid driving until effects are known.
- Dependence risk: Long term use can lead to physical dependence, requiring gradual dose reduction when stopping.
- Kidney function: Reduced kidney function requires a lower dose since clearance depends entirely on the kidneys.
- Respiratory depression: Combining with opioids, benzodiazepines or alcohol can dangerously slow breathing.
- Mood changes: Report new depression, anxiety or unusual thoughts to your doctor without delay.
- Pregnancy and breastfeeding: Use only when clearly needed and under close medical supervision.
- Withdrawal seizures: Sudden discontinuation can trigger seizures even in people without epilepsy.
- Elderly patients: Older adults are more sensitive to sedation and fall risk at this strength.
