How It Works

Norethisterone is a synthetic progestogen that binds to progesterone receptors in the uterus, ovaries, and hypothalamus. In the uterus, it causes secretory transformation of the endometrium (the uterine lining), opposing estrogen-driven proliferation and stabilizing the endometrial tissue. This reduces heavy bleeding and irregular shedding. In the hypothalamus and pituitary gland, Norethisterone suppresses the pulsatile release of gonadotropin-releasing hormone (GnRH), which in turn reduces luteinizing hormone (LH) and follicle-stimulating hormone (FSH) secretion, inhibiting ovulation. By maintaining a progesterone-dominant hormonal environment, Norethisterone prevents the endometrial breakdown that triggers menstruation, which is the basis of its period-delaying effect.

Period Delay: What to Expect

When taken as directed for period postponement, at 5mg three times daily starting 3 days before the expected onset of menstruation, Just-N maintains the progesterone signal that prevents endometrial shedding. Menstruation typically begins 2 to 3 days after the last tablet is taken, as Norethisterone levels fall and the endometrium sheds. The delayed period may be heavier or more prolonged than usual, which is a normal and expected physiological response. Prolonged delay beyond 17 to 20 days of continuous use is not recommended due to the risk of breakthrough bleeding and endometrial instability.

Clinical Note

Norethisterone has partial androgenic activity due to its structural relationship to 19-nortestosterone. At the 5mg dose used in Just-N, this androgenic effect can cause acne, oily skin, and mild hirsutism in susceptible patients. Norethisterone also carries a class-wide progestogen risk of venous thromboembolism (VTE), particularly in women who smoke, are above 35 years, are obese, or have a personal or family history of blood clots. The thrombosis risk is lower than with combined oral contraceptives but is not negligible. Just-N should never be used during pregnancy as progestogens in early pregnancy have been associated with rare cases of genital abnormalities in female fetuses.

How to use Just-N Tablet 5mg?

• Just-N should be taken with food or milk to reduce the risk of nausea.
• The tablet should be swallowed whole with a full glass of water at the same time each day.
• For period postponement: 5mg three times daily starting 3 days before the expected period, continued until the period is no longer needed to be delayed. The period will begin 2 to 3 days after stopping.
• For heavy or painful periods: 5mg two to three times daily from day 19 to day 26 of the menstrual cycle. Treatment is typically maintained for 3 to 4 cycles.
• For endometriosis: doses and duration vary and must be individually prescribed by a gynecologist. Typical starting doses range from 5mg twice daily with gradual escalation as directed.
• The dose should not be changed or the course extended without consulting a doctor. Prolonged unsupervised use carries risks of endometrial instability and hormonal side effects.

Drug Interactions

All current medicines, supplements, and herbal products must be disclosed to the prescribing doctor before starting Just-N. Key interactions include:

• Liver enzyme inducers such as rifampicin, carbamazepine, phenytoin, and St. John's Wort: These accelerate the metabolism of Norethisterone, reducing its blood levels and potentially reducing its effectiveness for menstrual regulation.
• Ciclosporin: Norethisterone inhibits ciclosporin metabolism, raising ciclosporin blood levels and the risk of nephrotoxicity and neurotoxicity.
• Anticoagulants such as warfarin: Norethisterone may alter the anticoagulant effect of warfarin. INR should be monitored in patients on concurrent anticoagulant therapy.
• Antidiabetic medicines: Progestogens can impair glucose tolerance. Diabetic patients may require dose adjustments of their antidiabetic medicines during treatment.
• Other hormonal medicines: Concurrent use with other progestogens or estrogens should be under specialist supervision to avoid unpredictable hormonal effects.

Risks and warnings

• Thromboembolism: Norethisterone increases the risk of venous thromboembolism (blood clots in the veins). The risk is higher in women who smoke, are above 35 years, are obese, have a history of clots, or are immobile for prolonged periods. Treatment must be stopped immediately and emergency care sought if symptoms of a blood clot develop.
• Pregnancy: Just-N must not be taken during pregnancy. Norethisterone can cause virilization of female fetuses (masculinizing effects on the developing genitalia) if taken in early pregnancy. A pregnancy test is recommended before starting treatment if there is any possibility of pregnancy.
• Undiagnosed bleeding: Just-N must not be used to treat vaginal bleeding that has not been properly diagnosed. All abnormal bleeding must be investigated by a doctor before starting hormonal therapy.
• Liver disease: Norethisterone is metabolized by the liver. It is contraindicated in severe liver disease and should be used with caution in any patient with impaired liver function. Liver function tests should be performed before long-term use.
• Breast and endometrial cancer: Just-N is contraindicated in women with known or suspected hormone-sensitive cancers. Norethisterone should not be used without specialist oncological assessment in women with a personal history of breast cancer.
• Diabetes: Progestogens can impair insulin sensitivity. Blood glucose should be monitored more closely in diabetic patients during treatment with Just-N.
• Depression: Progestogens including Norethisterone have been associated with worsening of depression. Women with a history of depressive disorders should be monitored carefully during treatment.
• Not a contraceptive at 5mg: Just-N 5mg does not provide reliable contraception. Women who do not wish to conceive must use additional non-hormonal contraceptive methods during treatment.
• Breastfeeding: Norethisterone passes into breast milk. The effect on the infant is not fully established. A doctor should be consulted before using Just-N during breastfeeding.

Just-N Tablet Side Effects

Most women tolerate Just-N well for short treatment courses. Side effects are more common with longer treatment durations and at higher doses.

Serious side effects are rare. Medical attention should be sought immediately if any of the following occur:

• Signs of venous thromboembolism including calf pain and swelling, sudden shortness of breath, or chest pain.
• Severe headache, visual disturbances, or sudden neurological symptoms, which may indicate cerebrovascular events.
• Jaundice or severe abdominal pain, which may indicate liver involvement.
• Signs of a severe allergic reaction including facial swelling, difficulty breathing, or widespread rash.
