What Experts Say
Dermatologists consider permethrin 5% the first choice treatment for scabies since it disrupts mite nerve function while causing very little skin irritation. Cure rates are consistently reported above 90 percent after one correctly applied dose, which is why most skin specialists prefer it over older sulfur-based ointments that stain clothing and smell strong.
Scabies remains common due to crowded living conditions, shared bedding, and warm humid weather that helps mites survive on fabric longer. Physicians usually advise applying the lotion to the entire body from the neck down, leaving it on for 8 to 14 hours before washing off, since mites hide in skin folds and under nails that a partial application would miss.
Doctors stress treating every household member and close contact at the same time, even those without symptoms, since untreated carriers reinfect the household within days. Itching can linger for two to four weeks after the mites die as the skin reaction settles, so specialists advise a follow up review if new burrows appear after two weeks.
