What Experts Say
Duloxetine works by blocking the reuptake of both serotonin and norepinephrine in the brain and spinal cord, which raises the level of these chemical messengers available at nerve synapses. This dual action explains why the drug lifts mood and also dampens pain signals traveling through the spinal cord, setting it apart from drugs that act on serotonin alone.
Clinical response for depression and anxiety usually takes two to four weeks of steady dosing to become noticeable, so doctors advise against judging the effect too early or stopping the medicine abruptly. Sudden discontinuation after regular use can trigger dizziness, nausea, and irritability, so the dose is tapered down gradually under medical guidance.
Because Duloxetine is processed through liver enzyme pathways, doctors screen for existing liver disease and heavy alcohol use before starting therapy, and monitor blood pressure since the drug can cause a mild rise in some patients. Combining it with other serotonergic drugs needs caution given the added risk of serotonin syndrome.
