What Experts Say
Clarithromycin remains a common first choice for chest, throat, and sinus infections in Pakistani clinics, mainly because it works well against the bacteria that usually cause these problems and is available as an affordable generic tablet like Macpar rather than a costlier imported brand.
Doctors caution that clarithromycin can affect heart rhythm and interacts with several widely used medicines, including certain cholesterol lowering statins, blood thinners, and heart rhythm drugs. A careful review of a patient's current medicines and medical history is recommended before starting treatment, especially in older adults or those with existing heart disease.
Specialists generally recommend finishing the entire prescribed course, usually seven to fourteen days, rather than stopping once symptoms start to feel better. Stopping too early raises the risk of the infection returning or the bacteria becoming resistant to future antibiotic treatment.

Drug Interactions
- Statins: Simvastatin and lovastatin should generally be avoided together with clarithromycin due to a higher risk of muscle damage.
- Warfarin: Clarithromycin can strengthen the blood thinning effect, so clotting levels need closer monitoring.
- Digoxin: Blood levels of digoxin can rise to unsafe levels when taken together.
- Carbamazepine: Clarithromycin can raise carbamazepine levels, increasing the chance of side effects.
- Ergot Medicines: Combining with ergot based migraine drugs can cause severe blood vessel narrowing.
Risks and warnings
- Heart Rhythm Changes: Clarithromycin may prolong the QT interval, raising the risk of irregular heartbeat in people with existing heart conditions.
- Liver Problems: Rare cases of liver inflammation or jaundice have been reported, so unusual tiredness or yellowing skin should be reported promptly.
- Statin Interaction: Combining with certain statins can raise the risk of serious muscle breakdown, so doctors often pause or adjust statin doses during treatment.
- Kidney Impairment: A lower dose is usually needed in patients with reduced kidney function.
- Pregnancy: This medicine should be used only if the benefit clearly outweighs the risk, since animal studies have raised some safety concerns.
