What Experts Say
Morphine sulphate is the reference opioid for severe pain that does not respond to weaker analgesics. It binds to mu opioid receptors in the brain and spinal cord, blocking pain signal transmission and altering how pain is perceived. This mechanism makes it effective for deep, persistent pain linked to cancer and other severe conditions.
The modified release form dissolves slowly inside the capsule, releasing morphine over several hours instead of all at once. This keeps blood levels steady across a 12 hour interval, reducing the peaks and troughs seen with immediate release opioids and lowering the chance of breakthrough pain between doses.
Morphine carries a real risk of tolerance, dependence and respiratory depression, so dosing is individualized and reviewed regularly by the physician. Lower starting doses are generally used in elderly patients and those with reduced kidney or liver function, balancing pain relief against long term opioid risks.
