What Experts Say
Esomeprazole is the active S isomer of omeprazole and works by irreversibly blocking the proton pump on stomach lining cells that produces acid. A single dose taken before a meal can suppress acid production for close to 24 hours, since the pump only becomes active once food stimulates it.
The 40mg strength is typically used for initial healing of erosive esophagitis and for confirmed severe acid related damage, while a lower 20mg dose is often continued afterward for maintenance once symptoms settle. Taking the capsule on an empty stomach thirty to sixty minutes before breakfast gives the strongest acid suppressing effect.
Doctors periodically reassess whether long term proton pump inhibitor use is still needed, since prolonged therapy is linked to lower vitamin B12 and magnesium levels and a higher fracture risk with extended use. Esomeprazole also reduces the effectiveness of clopidogrel, so patients on that medicine are usually switched to an alternative acid reducer.
