What Experts Say
Meloxicam belongs to the oxicam group of nonsteroidal anti-inflammatory drugs. It blocks the cyclooxygenase-2 enzyme more than cyclooxygenase-1, which lowers prostaglandins responsible for pain, swelling and fever. This relative COX-2 preference is why meloxicam tends to be gentler on the stomach than older non-selective NSAIDs, though gastrointestinal risk is never fully removed.
Trials in osteoarthritis and rheumatoid arthritis show meloxicam 15mg once daily gives pain relief similar to other NSAIDs, with a long half-life that supports single daily dosing. Steady drug levels build over about a week of regular use, so full anti-inflammatory benefit develops gradually rather than after the first dose.
NSAIDs as a class carry cardiovascular and gastrointestinal caution that rises with higher doses and longer use. Experts recommend the lowest effective dose for the shortest period needed, with periodic kidney and blood pressure checks for patients on long term therapy for chronic joint disease.

Drug Interactions
- Warfarin and Other Blood Thinners: Combined use raises the risk of serious bleeding.
- ACE Inhibitors, ARBs and Diuretics: This combination can reduce kidney function, an effect sometimes called the triple whammy.
- Lithium: Meloxicam can raise lithium blood levels toward toxic range.
- Methotrexate: Reduced clearance of methotrexate can increase its toxicity.
- Aspirin and Other NSAIDs: Taking these together adds to gastrointestinal bleeding risk without extra pain relief benefit.
- Corticosteroids and SSRIs: Both classes further raise the chance of stomach bleeding when combined with meloxicam.
Risks and warnings
- Cardiovascular Risk: Long term or high dose use may raise the risk of heart attack or stroke, particularly in patients with existing heart disease.
- Gastrointestinal Bleeding: Stomach or intestinal bleeding can occur, sometimes without warning symptoms, especially in older adults.
- Kidney Function: Meloxicam can reduce kidney blood flow, so caution is needed in patients with existing kidney disease or dehydration.
- Pregnancy: Should be avoided after 20 weeks of pregnancy because it can affect fetal kidney function and the closure of a fetal blood vessel.
- Allergy History: Should not be used by patients who have had asthma, hives or a serious reaction to aspirin or other NSAIDs.
