What Experts Say
Doctors prescribe meloxicam for osteoarthritis flares and for long term control of rheumatoid arthritis and ankylosing spondylitis because its longer half life allows once daily dosing, unlike older NSAIDs that need multiple daily doses. The 7.5mg strength is usually the starting dose, with an increase to 15mg only if pain control remains inadequate.
Prescribers monitor blood pressure, kidney function and hemoglobin periodically in patients using meloxicam beyond a few weeks, since NSAIDs as a class carry cardiovascular and gastrointestinal risk that rises with dose and duration. Elderly patients and those with prior ulcer disease are usually kept on the lower 7.5mg dose.
Clinicians generally avoid combining meloxicam with other NSAIDs or prescribing it to patients with significant heart failure or advanced kidney disease, since the combined risk of bleeding, fluid retention and reduced kidney perfusion outweighs the analgesic benefit in these groups.
