What Experts Say
Metformin hydrochloride lowers blood sugar mainly by reducing glucose production in the liver and improving how the body responds to its own insulin, rather than by forcing the pancreas to release more insulin the way some other diabetes tablets do.
Because it rarely causes hypoglycemia on its own, physicians worldwide continue to prescribe metformin as the first choice medicine for newly diagnosed type 2 diabetes unless kidney function, heart failure or another specific reason rules it out.
Pakistan has one of the highest rates of type 2 diabetes in the region, and doctors commonly start metformin early alongside diet and lifestyle changes, adjusting the dose gradually over several weeks to reduce stomach related side effects. Periodic kidney function and vitamin B12 checks are recommended for patients who stay on metformin for many years, since long term use can slowly lower B12 levels in some people.
