What Experts Say
Sulfacetamide is one of the oldest antibacterial agents still used in eye care. It works by blocking a step bacteria need to make folic acid, which slows their growth rather than killing them outright. Combined with a steroid, it is often chosen for milder surface infections where inflammation is bothersome but the infection itself is not severe.
Doctors are more selective with sulfonamide based drops today because resistance has grown over the decades and because some people are allergic to sulfa drugs. A history of sulfa allergy, even to a tablet taken years ago, is usually enough reason to avoid this combination and choose a different antibiotic and steroid pairing instead.
As with any steroid containing eye drop, the bigger long term concern is not the antibiotic but the dexamethasone component. Extended use can raise pressure inside the eye and cloud the lens over time, so most prescribers set a clear stop date and ask patients to return if the eye has not settled within a week or two.
