Drug Interactions
- Calcium Salts: Should not be mixed in the same IV line, since precipitation can occur.
- Catecholamines (Epinephrine): Sodium bicarbonate can inactivate these drugs if infused together in the same line.
- Diuretics: Combined use may raise the risk of metabolic alkalosis and electrolyte disturbance.
- Lithium: Increased sodium excretion can lower lithium blood levels.
Risks and warnings
- Metabolic Alkalosis: Rapid or excessive correction can push blood pH too high, causing new complications.
- Fluid Overload: High sodium content can worsen heart failure or high blood pressure.
- Electrolyte Imbalance: May lower blood potassium and calcium, requiring close monitoring.
- Extravasation: Leakage outside the vein can cause tissue damage at the injection site.
- Kidney Disease: Requires cautious use and dose adjustment in patients with reduced kidney function.
Side Effects
- Metabolic alkalosis if overcorrected
- Swelling from fluid and sodium overload
- Low blood potassium or calcium levels
- Injection site irritation or tissue damage if it leaks outside the vein
- Muscle twitching or spasms from electrolyte shifts
