What Experts Say
Prazosin belongs to a class of drugs called alpha-1 blockers, which work by relaxing the smooth muscle in blood vessel walls and around the prostate and bladder neck. This dual action explains why it is used both for high blood pressure and for urinary symptoms in men with an enlarged prostate.
The first dose is usually given at bedtime because it can cause a sudden drop in blood pressure when standing up, sometimes called the first dose effect. Doctors typically start with a low dose and increase it gradually while monitoring blood pressure response.
Because it affects blood pressure quickly, patients are advised to rise slowly from sitting or lying positions, especially in the first few days of treatment or after a dose increase. It has also been studied for reducing nightmares related to trauma, though this remains an off label use in most cases.
