What Experts Say
Rifampicin inhibits bacterial RNA synthesis, Isoniazid blocks mycolic acid production in the cell wall, and Ethambutol disrupts arabinogalactan formation. Together these three mechanisms attack Mycobacterium tuberculosis from different angles, which lowers the chance that surviving bacteria develop resistance to any single drug.
Physicians in Pakistan commonly prescribe this combination for the continuation phase of Category I tuberculosis treatment, once the initial intensive phase with pyrazinamide is complete. Weight based dosing is standard, since underdosing risks treatment failure while overdosing raises the chance of ethambutol related optic toxicity. Tuberculosis remains a major public health concern, and fixed dose combinations like this one support directly observed treatment strategies nationwide.
Regular monitoring during treatment includes liver function tests, since both Rifampicin and Isoniazid carry some risk of hepatotoxicity, along with periodic vision checks to catch early signs of ethambutol induced optic neuritis. Physicians also counsel patients about harmless orange discoloration of urine, sweat and tears caused by Rifampicin, so treatment is not stopped unnecessarily.
