Natrilam Tablet Uses

- Used as a single tablet substitution for patients already stable on amlodipine and indapamide taken separately at the same doses
- Helps reduce the risk of stroke and heart complications linked to uncontrolled hypertension
- Suitable for long term daily blood pressure control
Natrilam Tablet Benefits
- Combines two proven blood pressure lowering mechanisms in a single once daily tablet
- Sustained release indapamide gives smoother, more consistent blood pressure control
- Improves treatment adherence by reducing the number of tablets a patient needs to take
- The amlodipine component also helps with chest pain (angina) in patients who have both conditions
What Experts Say
Indapamide is a thiazide like diuretic that lowers blood pressure mainly by relaxing blood vessel walls and mildly increasing salt and water loss. Amlodipine is a calcium channel blocker that relaxes arterial muscle. Together they target blood pressure through two pathways, usually giving better control than either drug alone.
Pakistan has a high prevalence of hypertension, and many patients are diagnosed only after complications such as stroke or kidney damage appear. Physicians often move patients from two separate tablets to one combination tablet like this once blood pressure is stable, since fewer daily tablets improves long term adherence.
Regular monitoring of kidney function, electrolytes and blood pressure is recommended during treatment, since both components affect sodium, potassium and fluid balance. Ankle swelling from the amlodipine component is known and usually harmless, though sudden or severe swelling should be reported to a doctor.

Drug Interactions
- Lithium: Indapamide can raise blood lithium levels and increase the risk of lithium toxicity, so this combination should be avoided or closely monitored.
- NSAIDs: Regular use of NSAIDs such as ibuprofen or diclofenac can reduce the blood pressure lowering effect and raise the risk of kidney problems.
- Potassium Sparing Diuretics And Supplements: Combining with potassium sparing diuretics or potassium supplements can raise the risk of high blood potassium.
- Simvastatin: Amlodipine raises simvastatin blood levels, so the simvastatin dose should not exceed 20mg daily when taken together.
- Strong CYP3A4 Inhibitors: Medicines such as ketoconazole or itraconazole can raise amlodipine blood levels and increase side effects.
- Other Blood Pressure Medicines: Combining with other antihypertensive drugs can cause an additive drop in blood pressure.
Risks and warnings
- Kidney Impairment: Use with caution in patients with reduced kidney function, since both components depend on renal clearance.
- Electrolyte Imbalance: Indapamide can lower blood sodium and potassium levels, so periodic blood tests are recommended, especially in elderly patients.
- Liver Impairment: Use cautiously in patients with liver disease, and avoid entirely in severe hepatic encephalopathy.
- Pregnancy And Breastfeeding: Not recommended during pregnancy or breastfeeding, since both components can affect the fetus or nursing infant.
- Gout: May raise uric acid levels and trigger a gout flare in susceptible patients.
