What Experts Say
Clonazepam works by binding to the GABA A receptor complex in the brain and enhancing the inhibitory effect of the neurotransmitter GABA. This increases chloride ion flow into nerve cells, calming excessive electrical activity that drives seizures and reducing the heightened arousal seen in panic disorder.
The 0.5mg strength is usually the starting or fine tuning dose for anxiety and panic disorder, since higher strengths are reserved for seizure control in epilepsy syndromes such as Lennox Gastaut and absence seizures. Clonazepam has a long half life, so its calming effect builds gradually and lasts for many hours after a single dose.
Because tolerance and physical dependence can develop with continued use, most clinicians limit continuous therapy to a few weeks for anxiety and prefer the lowest effective dose. Stopping the tablet suddenly after regular use can trigger rebound anxiety or withdrawal seizures, so any dose reduction should be gradual and supervised by a physician.

Drug Interactions
- Opioid Painkillers: Combining with opioids raises the risk of severe sedation, slowed breathing, coma, and death.
- Alcohol: Alcohol intensifies the sedative effect and increases the risk of dangerous CNS depression.
- Other Benzodiazepines: Using together with other benzodiazepines multiplies sedation and respiratory risk without added benefit.
- Antifungal Azoles: Ketoconazole and itraconazole slow Clonazepam breakdown and can raise its blood level.
- Antiepileptic Drugs: Phenytoin and carbamazepine can lower Clonazepam levels and reduce its effect.
- Sedating Antihistamines: These add to drowsiness and impair alertness when taken together.
Risks and warnings
- Dependence and Tolerance: Regular use of Clonazepam can lead to physical dependence and reduced effect over time, so treatment duration is usually kept short.
- CNS Depression: The tablet slows brain activity and can cause marked drowsiness, confusion, or slowed breathing, especially with other sedatives.
- Withdrawal Seizures: Stopping the tablet abruptly after regular use can trigger rebound anxiety, tremors, or withdrawal seizures.
- Pregnancy and Breastfeeding: Clonazepam should be avoided in pregnancy and breastfeeding unless the physician judges the benefit outweighs the risk.
- Elderly Patients: Older adults are more sensitive to sedation and fall risk, so a lower dose is usually preferred.
- Hepatic Impairment: Patients with significant liver disease need dose adjustment and close monitoring.
