What Experts Say
Olmesartan medoxomil belongs to the angiotensin II receptor blocker class and works by blocking the AT1 receptor, preventing the vasoconstrictive and aldosterone-releasing effects of angiotensin II. This mechanism allows blood vessels to relax and reduces overall peripheral resistance, which lowers blood pressure without the dry cough often seen with ACE inhibitors.
Dosing usually starts at a lower strength and is titrated upward over two to four weeks based on response, since the full antihypertensive effect of olmesartan is not seen immediately. Physicians recommend regular monitoring of renal function and serum potassium, particularly in patients with pre-existing kidney impairment or those on concurrent potassium-sparing diuretics.
Pakistan carries a high burden of undiagnosed and poorly controlled hypertension, which makes consistent adherence to ARB therapy clinically important. Doctors across Pakistan often pair olmesartan with lifestyle counselling and periodic blood pressure review to sustain long term cardiovascular protection.

Drug Interactions
- ACE inhibitors: Combined use increases the risk of low blood pressure, high potassium, and reduced kidney function.
- Potassium-sparing diuretics: Raises the risk of hyperkalemia when used together.
- NSAIDs: May reduce the blood pressure lowering effect and worsen kidney function, especially in dehydrated or elderly patients.
- Lithium: Olmesartan can increase lithium levels, raising the risk of toxicity.
- Diuretics: May increase the risk of symptomatic hypotension, particularly at the start of therapy.
Risks and warnings
- Pregnancy: Can cause fetal harm and must be stopped as soon as pregnancy is confirmed.
- Kidney function: Use with caution in patients with reduced kidney function and monitor serum creatinine regularly.
- Hyperkalemia: May raise blood potassium levels, especially with potassium supplements or potassium-sparing diuretics.
- Low blood pressure: Can cause symptomatic hypotension, particularly after the first dose in volume-depleted patients.
- Dehydration: Correct fluid or salt depletion before starting therapy to avoid a sharp drop in blood pressure.
- Liver impairment: Use cautiously in patients with significant liver disease.
- Bilateral renal artery stenosis: Avoid use as it can worsen kidney function in this condition.
- Elderly patients: May be more sensitive to blood pressure lowering effects and need closer monitoring.
