What Experts Say
Psychiatrists in Pakistan prescribe citalopram as a first line option for depression because it has a simple, once daily schedule and fewer drug interactions than older antidepressants. Doctors typically start at 20mg daily and review response after four to six weeks, since SSRIs take time to build their full effect on mood.
The dose ceiling of 40mg daily, and 20mg in people over sixty or those with liver problems, exists because citalopram can lengthen the heart's QT interval in a dose dependent way. Physicians order a baseline ECG in patients with existing heart disease before prescribing higher doses, and they check for other medicines that affect heart rhythm before adding citalopram to a treatment plan.
Doctors watch closely during the first weeks of treatment, particularly in younger patients, since antidepressants carry a documented risk of increasing suicidal thoughts before mood actually improves. Physicians advise against stopping citalopram abruptly, since a gradual taper avoids the dizziness and flu like symptoms that can follow sudden withdrawal.
