What Experts Say
75mg daily is the usual maintenance dose of dosulepin used for moderate to severe depression once a lower starting dose has already been tolerated. It is rarely the first dose a patient is given, because tricyclic agents carry meaningful cardiac risk when taken in excess of the prescribed amount.
At this higher strength, prescribers pay closer attention to blood pressure, heart rhythm, and anticholinergic symptoms such as dry mouth, blurred vision, urinary hesitancy, and constipation, since these effects tend to become more noticeable as the dose rises toward the therapeutic range.
Response to treatment is reviewed regularly at this dose, and abrupt stopping is avoided because dosulepin can cause withdrawal symptoms. Doses above this maintenance range are generally reserved for closely supervised hospital settings rather than routine outpatient care in Pakistan.
