What Experts Say
Clarithromycin is a semi synthetic macrolide that binds the bacterial 50S ribosomal subunit and halts protein synthesis, giving it broad activity against gram positive bacteria and several atypical respiratory pathogens. This mechanism underlies its use both as a standalone respiratory antibiotic and as one arm of triple therapy for H. pylori.
Physicians pair clarithromycin with a proton pump inhibitor and a second antibiotic, usually amoxicillin or metronidazole, when treating H. pylori linked peptic ulcer disease, since single agent therapy fails to clear this organism reliably. The 250mg tablet strength allows flexible twice daily dosing across both indications.
Because clarithromycin strongly inhibits the CYP3A4 liver enzyme, doctors review a patient's full medicine list before prescribing it, particularly statins, warfarin and certain heart rhythm drugs. This interaction profile, along with a rare risk of QT prolongation, is why the tablet remains a prescription only antibiotic rather than an over the counter option.
