What Experts Say
Ropinirole is a non ergoline dopamine agonist. It stimulates D2 and D3 dopamine receptors in the brain, which compensates for the dopamine shortfall that underlies both Parkinson's disease and restless legs syndrome, two distinct conditions that nonetheless share this same underlying receptor pathway.
In Parkinson's disease this receptor stimulation smooths out tremor, stiffness and slow movement, while in restless legs syndrome the same mechanism calms the abnormal crawling or aching urge to move the legs that typically worsens in the evening and at night when the limbs are at rest.
Neurologists usually start dosing low and raise it gradually over several weeks, since dopamine agonists carry a recognized risk of sudden daytime sleepiness and impulse control changes such as compulsive gambling, shopping or eating, risks that tend to appear more often once the dose climbs higher or when combined with other dopaminergic drugs.
