Simbex combines two medicines that lower cholesterol through completely different pathways. Ezetimibe works in the small intestine, where it blocks a transporter protein responsible for absorbing cholesterol from food and bile into the bloodstream.

Simvastatin works inside the liver. It blocks an enzyme called HMG-CoA reductase, which the liver needs to manufacture its own cholesterol. With less cholesterol being made internally, liver cells pull more LDL cholesterol out of the blood to make up the difference.

Because ezetimibe reduces the cholesterol absorbed from the gut while simvastatin reduces the cholesterol the liver produces, using them together lowers LDL cholesterol more than either medicine could achieve alone at a similar dose. • Certain antibiotics and antifungals: Clarithromycin, erythromycin, itraconazole and ketoconazole raise the risk of muscle damage
• Gemfibrozil and cyclosporine: These are usually avoided with Simbex due to a higher risk of serious muscle injury
• Amiodarone and diltiazem: These may require a lower simvastatin dose to reduce interaction risk
• Grapefruit juice: Large amounts can increase simvastatin levels in the blood
