What Experts Say
Azithromycin binds to the 50S ribosomal subunit of susceptible bacteria, blocking protein synthesis the organism needs to grow. Its long tissue half life allows short three to five day courses to clear infections that would otherwise need seven to ten days of other antibiotics. This is a major reason it remains a common choice for respiratory infections.
Completing the full course matters even after symptoms improve, since stopping early leaves surviving bacteria exposed to a low drug level. This is one of the clearest paths toward antibiotic resistance, a growing concern where self-medication with leftover antibiotics is common. A short course finished properly clears infection more reliably than one taken inconsistently.
Azithromycin only works against bacterial infections and has no effect on viral illnesses such as the common cold or flu. Prescribing it for a viral illness offers no benefit and only adds side effect risk. Doctors also weigh a small but real risk of heart rhythm disturbance, worth mentioning before starting treatment, especially in older adults.
